Below is the opened statement for review. You
will note the statement is at the top and a return envelope is attached to the bottom. There is a tear off stub which
provides the account number, patient name and payment amount and other accounting information needed to post the payment.

Typically, a statement bill has an outgoing envelope, the statement
form itself and a return envelope. This typical method involves three different types of paper that have to be produced
at three different manufacturing facilities.
The "innovative" statement
begins as one sheet of paper that is 8 1/2" x 14". This single sheet of paper becomes the outgoing envelope,
the statement and the return payment envelope. Imagine the savings
in energy when all you need is one product that does what three typically have done! Only one manufacturing facility
needs to produce this form instead of three.
Instead of three different items being produced, packaged and shipped
by different carriers to a specified location, there is only one item. The transportation cost and manufacturing electricity cost
are minimal for the "innovative" statement form/envelope verses the traditional method.
